The following provides a general description of our three-phase approach to enhancing care management capabilities. Phase 1 is a comprehensive opportunities and operations assessment of the care management program including utilization management, discharge planning and case management. Phase 2 is the development of a care management strategy and Phase 3 is the implementation of the strategy.
