developed a three-phase approach to enhancing care management
capabilities. Phase 1 consists of a comprehensive opportunities and
operations assessment of the entire care management program
including utilization, case and disease management
in addition to support areas such as network contracting and
reimbursement, quality, marketing and underwriting practices. Phase
2 provides the development of a care management strategy, followed
by Phase 3, the implementation of that strategy. The following
